Assistance with Social and Community Participation

Increased Social and community participation is one of the main NDIS goals that most of the people with disabilities have in their NDIS plan.

We at Sowilo disability services delivers these supports that focuses on our participants needs, experiences, interests and capabilities. We provide these activities with proper safety and control and try to make it enjoyable for our participants.

These Activities are important but are often taken for granted and neglected.

THESE ACTIVITES MAY INCLUDE THE FOLLOWING

  • Visiting parks, beaches and other attractions

Group and center-based activities

We at Sowilo Disability Services promote socialization. Our client’s choices and preferences are valued here.

Our clients are welcomed to undertake activities within our facility independently with complete safety and controlled environment.

ACTIVITES INCLUDE

Social and community access- for example drive, socialisation, etc.
Recreational activities – for example picnic, art and craft, drive, etc.
Developmental life skills – for example gardening, shopping, cooking, etc.
Improve health and well-being for example yoga, group and fitness activities, swimming, etc.
Promoting literacy and numeracy – for example reading, writing, money management, etc. 

Services

STA/ Respite care services

When you require short-term accommodation/ respite services for a break or to develop your abilities, short-term housing is available. This is possible for up to 14 days at a time. You can divide up the 28 days that the NDIS funds annually based on your unique goals and circumstances.

You can try new things, meet new people, and learn new skills during your brief time with us while also providing your informal supports a break.

Depending on what you want to accomplish when you return home, we assist you in attaining your goals on each visit.

Services

Support Coordination

Making the most of your planned supports is made easier with the aid of support coordination. Support coordination is a tool for creating capacity that aids in:

  • Recognize your NDIS plan and use it to further your objectives.
  • Link you to NDIS providers, community, mainstream services, and other government resources
  • Increase your self-assurance and coordination abilities when using your supports.
  • Services for support coordination are provided by a support coordinator or specialist support coordinator.

 

You can include one of the following three levels of support coordination in your strategy:

  • Support connection: With this support, you will be given the tools you need to establish connections with informal, community, and financed supports so you may make the most of your plan and work towards your objectives.
  • Support Coordination (Level 2): This assistance will help you develop and connect you with NDIS service providers, the community, traditional media, and other With this support, you can develop the abilities required to comprehend and apply your plan. A support coordinator will collaborate with you to make sure that a variety of supports are used to improve your ability to manage service delivery duties, maintain relationships, live more independently, and participate in your community.
  • Specialist Support Coordination (Level 3) – This is a more advanced level of coordination for support. It is intended for those whose circumstances are more complicated and who require specialised support. You will receive help managing difficulties in your support environment and maintaining consistent service delivery from a specialised support coordinator.
